Adding a new problem

Here you may add or edit a problem, asking the client for an appropriate label and capturing more detail if you wish. The date raised will default to the session date from which you accessed the problems list or from today's date if you accessed the problems list directly.

NOTE: It is important that the dated entered in the 'date raised' field corresponds with the relevant session date.

The length of time the problem has been an issue can be chosen from the drop-down menu.

Remember to tick 'problems' in the client profile and sessions so that problems will display on the session record


Problems, goals or solutions?

The labels and the detail may be used as client and practitioner wish. For example, if a more goal orientated approach is being taken the problem can be stated as a goal. Whether it is viewed as 'problem reduction' or 'movement towards the goal' the problems section may be used to track progress.

Tracking change on problem scores

We use the same traffic light alert system for tracking change on problem scores as we do for all other measures. 


Scrolling down the page, each problem can be reviewed by looking at the degree of progress since it was first identified on its own unique individual problem chart with relevant summary information.



In addition, the aggregate overall problems score is represented on the client chart.
